Have you a website with a survey image for OnDemand Research

Help needed please I am new to CPA and want to add the image for OnDemand Research to the website in my signature and I get the dreaded cross. I can work out why the code is wrong but not how to put it right.

If anyone has a working image I can read your source code if you please sent me the URL to your blog. I would appreciate any help I can get from this fab forum.I know how to read your source file so there is nothing more for you to do than send me the URL.
