Default Question, Getting Paid By Lead

Fellow Warriors,

Can anyone tell me what affiliate networks I can become affiliaited with that primarily have companies that are looking for people to fill out forms, register, answer questions, fill out applications, etc.. Such as insurance companies, credit card companies, organizations, educational institutions, research firms, etc., all wanting forms to be filled out or registrations, that pay per lead not necessarily per sale?
