Where do I place a CPA offer

If I have a e-book and squeeze page that req. Name and email address, where do I place a CPA offer ??

Don't you want a customer filling out as few a forums possable.

Is there any way you could customize this??

Hi Tucson,
You can place your banner creatives in your thank you page after a user fill his/her information in your squeeze page. And you can use the text link creatives in your ebook.

You should place the offer in the Google Adwords campaigns to get best possible traffic.
