Sales/Landing/Opt-in Pages on Separate Domains?

Three questions actually:

1) Do we need a separate landing/sales page for EVERY single CPA campaign we promote?

2) Do all the sales/landing pages need to be on their own domains? Or multiple landing pages on same domain for one niche (and split test them?).

3) Are these domains the ones we advertise through CPC or is it best to direct link to the CPA offer?

Any other insights you guys would like to offer this newbie?
