HELP ! Issues in Geo-based Redirect

Hi,

I have quite a number of Ads running.

The Issue :
There were 6 instances of ad clicks being redirected to ADULT SUPPLEMENT site with very lewd images. These happened for those living inside the targeted ad coverage. I do not know how many more were being redirected to such content.

Possible Implications to me:
1. Affects my credibility
2. Some Ads were declined during the approval process when they test-clicked from Canada.
3. I may get banned by my Ad Space Provider if this issue is not resolved

Therefore, I would like to mitigate this problem as soon as possible with the help of Warriors.

The facts:
1. Some Ads cloaked with TinyURL are facing this problem
2. Some Ads cloaked with Tracking202 are facing this problem.
3. Ads which were not cloaked did not go to the lewd sites but will go to Smiley Centrals for some countries.
4. I used free US-based proxy servers to test click my Ads
5. All Ads are for US coverage only.

My questions:
1. WHO CONTROLS THE GEO-BASED REDIRECTIONS for CPA Networks?
2. Will Tracking202 or TinyURL take over the redirection if the visitors were not allowed to reach the Ad Landing Page.
3. Are the Ad Servers of Advertisers/Merchants doing the redirections?
4. Has anyone else been facing the same issue?
5. What do I do?
