How to make a simple contact form

Hi

Can anyone tell me how to make a simple contact form such as:
Name
Tel:
email:
Colour:
etc:

Something like that and then they can click 'send'

Or alternatively, if I say 'Contact us for Free tree planting guide' then they click that and a email window opens how do I get that it says in the subject line 'free tree planting guide'?

Contact forms are rarely simple if you do not want them to be abused. There are a few standard ones that you can use depending upon what programming language you are using, e.g., PHP, Perl, etc.

As for the email link the following will work:
