Odd quality issues with a keyword in adwords

I'm having an odd issue with one of my keywords in my adwords account.

Keyword relevancy, landing page quality and loading times are "no problems". Yet my quality score is 4/10 for that keyword. Did I get slapped?

Also for that same keyword the status says "to prevent double serving, only one ad per advertiser may appear on a given page per keyword."

How can my ad be double serving when I'm using my own domain?

You're not slapped, you just got poor ads.

Never seen that message but my guess is you have the same keyword in different groups.

Yea, turns out I should've grouped those similar keywords together in the same adgroup instead of spreading them out into their own adgroups. Problem solved. Thanks.
