If you have a massive site use /%category%/%postname%/ & for smaller niche sites use /%postname%/

What i do on my sites is just change the custom tag to postname and i am done. I think that is SEO friendly but I also discovered that it is not possible to do on a windows hosting(in my experience) so i just stick with linux because of that.
