Can anyone tell me if the use of a pop up form against the terms of service of Adsense?

Yes, google wont like popup's

Sites showing Google ads should be easy for users to navigate. Sites may not change user preferences, redirect users to unwanted websites, initiate downloads, include malware or contain pop-ups or pop-unders that interfere with site navigation.

I think this is taken a little out of context. Specifically here this refers to pop-ups on your site that "interfere with site navigation". If a visitor can close popup without being redirected it should comply.

However, placing your adsense on popups or pop-unders is specifically prohibited by the Program Policies, See Ad Placement section.

Google does not say exactly that it is against Popups or Popunders, but it cleverly broadens its conditions by saying "Sites showing Google ads should be easy for users to navigate" .... The best is avoid them if u are using AdSense.
