Has anyone ever successfully had their adsense account reinstated after being banned?

I know of a few, though it's a pretty rare occurrence. I'd say if the account was banned for any 'decent' reason, the chances are very slim.

yes ... there are some terms and conditions involved in it. If you solve the reason for which you where banned and maintain a good profile for some time long then they do restore id's

its kind of hard to fix. due to the fact that google does not tell you what they didnt like
