Default Google and omitted content, what's the deal there?

Hopefully somebody can help me understand something about Google and omitted content.
I did a search for site:mysite.com to get indexed pages. Got about 226 results.

However many of those results are urls removed 6 months ago via Webmaster Tools for duplicate content reasons.

When I went through Google's SERP, I got this message:

"In order to show you the most relevant results, we have omitted some entries very similar to the 137 already displayed. "

The omitted result showed all urls requested to be remove and was approved.

Are these indexed pages or cached pages or what?
Does Google ever really remove pages you request?
Finally, since pages I requested to be remove are still there, is this a duplicate content problem?

Google tend to hide supplement results from the search results. These supplement results are almost similar to the displayed results (or you can say duplicate results)

Whenever Google hides supplement results, a message like this gets displayed at the bottom of the page.
