SEO Pressor Question

I have a question about SEO Pressor.

When you add a keyword, why does SEO Pressor only sometimes bolden the keyword? I notice that it always italicizes and underlines the keyword, but only boldens it sometimes.

I know the reason isn't because there isn't enough instances of the keyword because it does this even when you have plenty instances of the keyword and a good density.

My second question is related to on page SEO in general.

When you bolden, italicize and underline your primary keyword, does this have to be done in any particular order on the page or doesn't it matter? i.e. should you bolden the keyword before italizing it.
