ePCM Rates Poll & Best Practices

I read some news that online ad rates have been falling. I am working on a sports site that gets good traffic but ePCM are well under $1. Can this be corrected with ad placement, or does that matter so long as the ad does not violate the ad's TOS? I assume for Adsense it might since revenue is a mix of CPC and impressions?

Effective cost per thousand ad impressions (ePCM) - is this the same things as PCM right?

OK, am I confusing CPM and ePCM or something? I hear from some folks the rates are dependent on CTR and other folks demographics, does this vary from network to network?
