I don't include the blank jpeg, but I do include the psd file with the layers. That way you can remove the text or any other layer easily and replace it with your own.

In regards to domain names, I would do a search for a domain name that has a lot of searches (exact) and that doesn't have too much competition so which would be easier to rank for.
