"Jump through hoops" copywriters?

For an article I'm writing for a major blog, I am looking for two examples of copywriters who have a page where they make prospective clients jump through hoops to become clients, either by having to fill out a questionnaire or in some other way.

      Thanks! I am not sure I can use David's example, though, because he only "recommends" someone fill out the questionnaire. He doesn't require it, and therefore I'm sure quite a lot of lazy potential clients simply skip it and call him. Hence I am not sure this is a "jump through hoops" example.

            No Marcia.

            Ray Edwards from Spokane use to have a form
            clients had to fill in, or 'jump through hoops' on his website.
            You'll have to check to see if he still does.

    I have a questionnaire I ask clients to fill out. It's not to "become" a client. It's so I'll have the information needed to give them a quality sales letter. I've had one complaint from someone saying it was too long and the people they find on fiverr don't make him fill out a long questionnaire.

      I have a questionnaire I ask clients to fill out.
      Thank you, but it is not the questionnaire per se that I'm looking for, but the questionnaire as a way to put up a barrier to becoming a client and weed out people who are unsuitable as clients.
