Review form on e-commerce sites

Marketers, we know that the reviews it is important.
But it would be useful for us in the review form? Actually the field where we leave a review. Ratings, the number of stars. To share in social networks. Add videos and photos to the review (although, if you shoot how you use product and want to show). Pros and cons, so you can add your pros and cons.
Something else?
Or is it enough the review field?

Also, for the buyer it is important the responsive design of the reviews form. A client use on the various devices.

Shop owners: buyers will write a review and be happy. Remind them to e-mail two days later? Discounts? Or they need recognition?

What the form of reviews should or can has, which fields and functions?

    Hi Alex,

    We ask for reviews from all purchasers a few days after that have received their products.

    This is done automatically by different software depending on the shopping cart platform we are using.

    One I particularly like is "Order Reviews" by Isenselabs for OpenCart.

    Regardless of what CMS you are using you would get a lot of benefit by having a look at how that system works.

    People review right from their email and don't have to take any complex actions to post you a great review and then you can trigger a rewards program.
