  I am using PrestaShop Ecommerce.
  
  I have seen few PrestaShop modules through which I can add extra fields on checkout page like I have Goggled this plugin http://www.fmemodules.com/en/51-custom-fields.html . But I am not sure that should these plugins fulfill my requirement.

  These things should be done AFTER checkout is completed NOT before. Every additional field you require during checkout hurts conversions and distracts customers from finishing the transaction.
  
  Put these things on the order confirmation page for best results.
