Crap. Just wrote a post about Leadpages. Got that mixed up with GetResponse, haha.

Anyway, what exactly is the problem you're having?

Replace the existing index.html file with the one attached.

But make a copy of the original first, in case I didn't do what you wanted.

I assumed you want the script that optionally uses facebook info, if the user has an account and is logged in?
It seems a little slow to load.

Also, I obviously didn't test it.

Thanks but I wanted the form to be as the original. What you did looked like this when loading:
