Where to place my lead capture page in wordpress blog?

Hi warriors,
I am really lost after setting up my lead capture page, I don't know where should be the best place for it in my site.
Is it better to make it the first page after someone visits the blog? Or should I place a link in the menu? Or a banner in the header??
What do you think guys?? Any ideas?

UPDATE: I changed my lead capture page to a subdomain instead of a blog page with some changes.
I used the smart bar to link the lead capture page from the blog as you can see in any post and hide it from the homepage because I already set up a popup to show only in the homepage with the exit intent.
What do you think warriors, any suggestions??

    You landing page is okay.. You no need to add any pages ..

    Just you need make linking every page in your website..

    In your blog page or any pages posts just linking with this page

    My suggestion is too make the opt-in form small and on the top right. I say this becasue all your content is below the fold and not visible unless you scroll down, a visitor may never see it. If the form was in the top right area your content could begin near the top as well. You want to make it visible so a visitor sees more right away.

      1 - 2 competing call to actions (submit) and big button (No thanks go to blog) ! comon ! Why do you make no thanks a bigger button with similar color !

      2 - You need new logo

      3 - Header font is silly.

      4 - Book title on book cover is not visible

      5 - Remove Navigation links from footer (Home - about..etc)

      6 - Definitely through away Bluehost Advertisment.

      I'd focus more on benefits of YOUR book rather than the history of affiliate marketing. But is interesting though..Try some mix and match.

        I suggest you to look at some landing pages by LeadPages. Don't buy just look at them, maybe you'll get some idea.

          Thank you guys for your precious feedback, this is my first design ever. I am using the JustLanded wordpress theme. but i find it very complicated. I'll try to see other pages to have a better idea. thank you
