Wordpress Plugin to capture email from quiz

Hi all, i am new to affiliate marketing

After months of research i found out one of the efficiency way to capture emails is give target customer a quiz.

I having a wordpress website and i would like to create a quiz:
-Customer will do a quiz
-Enter email to receive result
-Send an email to customer with the link to the result
-The result page is also a landing page that i can sell my product
-Send some more email to close sale

Example:
The quiz page: "Can I Get My Ex Back?" Take The Free Quiz & Find Out!
The result page : QUIZ: Your Results Report

I think i need a quiz plug in and also a getrespone intergrated with it.

Anyone can give me the name of the plug-in above ? i tried some plug-in but its doesn't have the same function like example above, can any one give me a name of that plug in ? or suggest plug in similar to that

      i Tried all of them but none get the same funtion which generate a unique ID and unique result page

      I also did searching for the plug-in on that page but i can't find it.

      My finally solution is email the web owner but he haven't answer yet.

    I am wondering if this is the only solution on the market. Any other one?

    I would like to create a quiz for lead generation for my Wordpress website. About 10 Yes/No questions and opt-in for results.

    It's important that each response creates specific predefined analysis and all of them create a customized result webpage for a quiz-taker. Remember Eben Pagan's Psychic Sales Letter?

    Anybody knows about some plugin/software to do what is shown in the video? That each response creates specific predefined analysis and all of them create a customized result webpage for a quiz-taker. Or something similar.
