Could someone please give me a quick analysis of my optin form?

If you recommend a change or form, where/how should I go about getting the change?

    Mitch, sorry but it's not good!
    First observation is that it doesn't look professional. Second, you have added too many points there. Third, you should add an incentive for potential subscribers to give you their email.
    Fourth, no need to mention the word "optin" on the left top.
    I would suggest you to look for someone to be in top of your email marketing campaign.

    My two cents...

    1. Make sure you strike a pain point or problem area that you are going to solve as a headline for your page. (e.g. - Are you tired of cowering from across the room desperately pining for girls to notice you?)

    2. Find a way to empathize with the person reading your page, then...

    3. Add a small handful of bullets that not only talk about the benefits of your offer, but be more pointed at each bullet picking at their pain points from the headline.

    4. make the next progression your simple call-to-action for providing their name and email. Just be clear and to the point of what you want them to do next and what they can expect to get on the other side.
