Anyone else having a problem installing John Taylor's Easy WP Affiliate Plugin on their Wordpress blog?

I installed this plugin a couple months back with no issues but tried to install recently on a couple new blogs but it 'ain't working'.

This is a nifty little plugin and would love to get it up and running again.

Anyone have a solution?
