Looking for a special wordpress theme

that will let you put a video in the banner along with an opt in box. Does anyone have one of those?

It also needs to be super easy to set up.

This maybe a little OTT for what you're looking for but Squeeze Boss might be worth a sneaky peak.

It's basically slaps a fully fledge squeeze page on the top of every page of your Wordpress site. I use it on Autoblogs in markets I'm listbuilding in and I'm having great success with it.

Here's one of my sites I'm using it on so you can get an idea how it works.
