Need help with custom opt-in form

Can someone help an HTML challenged soul like myself? I downloaded a template for an opt-in form and tried to edit and upload it and get it to work with my Getresponse list. 2 problems. The tables are all screwed up and submitting the form returns an invalid email address error. If this kindof stuff is a piece of cake for you, can you have a look and offer your suggestions?
