wordpress blog just randomly stopped creating new pages :-\

Hey Warriors,

I've been working on my website (it's a wordpress blog) and the last page I created won't display properly. It's weird because I didn't do anything different.

Here's the page that's not displaying properly:

Index of /forms

As you'll see, all the others work perfectly -- (Ear, Nose, Throat & Facial Plastics | Rogue Valley Ear, Nose, Throat & Facial Plastics).

Anyone know why this would be happening?
