Tech/HTML Veterans... Could you lend me a hand?

I'm really up the creek on this one, one of my sites One time offer page isn't displaying all of sudden since I uploaded a revised version...
I'm sure it's only like a 2 minute fix but I'm stuck, if anyone is good at HTML (if you're familiar with the butterfly marketing script even better) I'd really appreciate the help.
