Wordpress Plugin Order Form Help

INTERNET MARKETING

I am looking for a wordpress plugin to create an order form, I want it to have these features :

1) PayPal integrated
2) Ability to add coupons
3) One time fee, I dont like paying monthly
4) Preferably integrates with mailchimp

I would truly appreciate if someone can help me out, I have looked everywhere for this.

I'm not sure if you need a shopping cart or order form. I use wpEstore on all of my sites. It is a full shopping cart WP plugin. It does all 4 of your requirements.

They also have an amazing affiliate program plugin when you are ready for it.
