Increasing Page RPM w/ AdSense

I am running AdSense exclusively on my site. I am also getting 5-10k pageviews a day on average, but my RPM is always only $3-$5.

Obviously if my RPM was close to $10 or more, it would substantially help my income. What do I need to do to get $10, 15 or $20 for my RPM? I've seen $20 RPM before on my site, but I never figured out how to maintain that.

I have one 300x250 Ad that is always in the top-left corner of the blog, and I always place one 336x280 in the blog post, along with a 'responsive' ad in the text post as well.

    Adsense usually gives you tips and suggestions for increasing earnings. Speed up page load time, use text/image ads as well as link units.. These are just a few things you can do to help increase revenue.
