Where to get an OTO page script for upsell of audio ebook?

Does anyone know where to get a script for selling an audio version of the ebook that I have just sold to the customer?

Many thanks for anyone who knows where I get can a good script(copy) for this.

I've an oto script but I couldn't find where I downloaded it from, it came with free give away rights so I think I can give it to you. PM me your email ID and I'll send it right away.
