Website creation question

I'm trying to figure out how to ad a page to my website that will be full of fields for an applicaton page. I have no idea where to go.

Prefer (Site 1) (Site 2) (Site 3) (Site 4)

Click here to pay by Credit Card

This is just a ruff example of what I would like to do with interactive fields and a way to register there email and have a record of each one that signs up in my affiliate programs.

Any ideas

Is this for Wordpress, an html site, or what?

You may also try elance. You'll find someone to install a form for under $50 with all the bells and whistles.

It does depend whether it's a regular html site or a wordpress site as well,

If it is an HTML site then you will need to use the Form tags and add different elements such as text boxes, radio buttons, etc.

Go to the following site and then click into the HTML Forms tutorial (sorry cant post links yet)
