Is it possible to put an opt-in form on your squiddo page?

No its not. You could put an image of one and when they click on it, have it take them to a page that you control though where you have an opt-in-form.

Yes you can...

The formatting was screwed when Squidoo changed its styles, and now I can't edit the lens because they consider it "SPAM".

Long-story-short... yes its possible to add an opt-in form to your lens (unless things have chcnaged since I did this a few months ago).
