Anyone know of a good Quick Contact Us Form??

I need a simple form to allow my visitors to contact me without emailing me. I've been searching google keep finding scripts from 2004 and beyond. Looking for something newer and that has spam blocking features like the verification code you type before submitting your message.

I just was searching Google and found these two possible forms;

Contact PHP Email Form Script w copy n paste free contact php script code - looks a little old, but says it blocks spam using PHP.

Email Me Form™ - HTML Forms, Web Forms, PHP Forms - This one you have to sign up for, I think its free. But it looks like it uses the verification code images.
