About me questionnaire?

Hi guys
I'm currently building a website for a friend in a different country...
its not easy to communicate back and forth so i wanted to send him a questionnaire to fill out.

My question is, does anyone have a template i could use? it would be a simple
About me questionnaire in relation to his services etc...

    This can be created via google documents and then you can just send him a link

    What you could do is go to surveymonkey or a survey site (have a free membership option...or at least use too). This way you can make a quick survey (extremely easy) with whatever questions you're looking to get answered. On top of that, it will automatically save the template so you can reuse it with other people.

    Surely a quick session in Skype would clear all the questions? It's quick, you can clarify all the answers and get it all done within an hour or so?

      Yea a quick Whats-app questionnaire was the way to do it. Latin Americans use whats-app instead of Skype. i realized this. thanks!
