I have been useing Trellis Desk for years, and have never had any issue with it, it's free and very robust.

Check it out:

A lot of web hosts provide you with help desk scripts... Check your cPanel and see if you have Fantastico... In there you might find that you already have free help desk scripts that you can use.

Surprised no one has mentioned Kayako.

I've used it for a few years and found it to be excellent

Perldesk is another one that is very useful, though to be honest we found it to be a tad clumsy to use at first, (Why we changed to Kayako)

I know some folks swear by Perl tho
