Help! How Do I customise Getrespone opt in form

Hi,
Please I need help on how to customise opt in form on my wordpress blog to my taste. I've seen good looking opt in form on several websites I wonder how they did it.

Can anybody help me in knowing how to do this.

Most nice ones are custom coded. However, you can get a decent one rolling with the FasterIM plugin or the Codebanter plugins.
