Ebook disclosures

I'm about to launch a product and noticed that recently every report I buy has about a half page of disclosures. Does anyone know where I can find a boilerplate sample of these that I can legally use?

Basically, the "don't rip my report off and sell it on the black market" and "I can't guarantee you'll become a millionaire" disclosures are the most common I'm seeing these days.
