Question About CPA Landing Page And Building A List

Hey Everyone,

I'm trying to create a landing page for one of my CPA offers. I also want to create and opt-in list with the traffic. How do you go about having both of these actions on the same landing page? I don't understand how you could set up both since each one requires a separate action (click banner for CPA offer or fill out opt-in form). I realize I'm probably missing something. Would someone mind to explain to me how I can make this happen?
