How do PR agencies work?

Does anyone know how PR agencies bill their work or how they are paid? For example, if I hire a PR agency to get me in a publication, does anyone have a ball park idea of how much they cost, how they work (do you only pay if you wind up published somewhere?), etc.? It's hard to find this info anywhere, really curious though.

    Andrew, have you check out PRWeb? I know it's not an agency but you may find it more useful.
