Zipper Opt In Form? How do I get it?

I recently saw an email opt in form design that I really liked. I'm not sure what it's called but it's an arrow in the top right corner of the website that when you hover over turns into an email opt in form. I'm using Aweber right now so I'm wondering how I can get that for my own website...

If you know of any websites that are currently using this form please let me know, or if you know what the script is called that would be extremely helpful.

    No one else has seen this? I've been looking all over for it for the past hour. I've seen slide-ups before, but this one opens up when someone hovers over the arrow.
