Affiliate Sales and Phone Numbers

As an affiliate, I think I understand that a cookie is set when someone clicks on my link on my web page, etc. to get to the merchant web site. Obviously, there's a certain level of trust involved on my part.
Here's my newbie question: when the merchant offers an 800# to affiliates for them to pass along to potential customers for questions, more info, or to order, how are those sales tracked? Does anyone else feel a little uncomfortable with that approach? Or am I just being too paranoid?
What questions should I be asking the merchant?

    What I've always done is use PHP to read in the affiliate id from the link, stick that underneathe the phone numbers on the sales page as a "Reference Number" and have customer service ask for it when someone calls in to place an order. The affiliates love it. I think this is a much better approach.
