What Payment Processor Is This?

Hey everyone,

This is going to be like a bizarre game of charades as I try to explain a payment system that I've seen on a few websites and you guys and gals try to work out what I'm talking about!!

Imagine, if you will, that a sales page has a multi-step order system. It begins with a specific opt-in style form and then following the opt-in you are taken to the payment form and then presented with the up-sells etc etc...

Here is the first step of the three:



Any ideas as to what software is running this? I've seen it on a few Internet Marketers sites....
