Contact Me Form?

I am trying to find a reliable "contact me" form to put on my blog - anyone know of any? I tried uploading one once but it didn't work! I wish I remembered which one it was...

Why don't you try a helpdesk instead. Easy to manage all contacts in one place.

Looking for a simple solution other then using Matts formmail with HostGator for an order form

Cforms II is what I use also on some of my Blogs.

I like plurk a little better than twitter but I use both! We're ahead of the times.
