Good Submission / Contact Forms Script?

Hey Guys,

Do you know of any good sites or source where I can get my hands on a script or 'software generator' that generates Submission Forms?

Basically, my potential clients visit my web page and fills in some details in a form like:

- Field Name: Name, Email Address, Contact No. etc
- Text Area: describe some things about him or herself depending on the question
- Tick Box and Radio Button

When the person hits submit, the form will be sent to my Inbox so I can take the appropriate follow-up action.

Any good scripts you know can get the job done? Thanks!

    Most help desk scripts are capable of this. If you have cpanel, there is support ticket, php ticket, have a look at fantastico.... blue smiley icon in your hosting panel. These are all free.

    Frank Haywoods help desk script is one I would reccommend if you are looking for a paid script. PM me if you'd like the url

    Thanks but I'm not looking for a helpdesk script. I have one already in fact, PerlDesk.

    What I'm looking for in specific is a Submission Form that features details for the visitor to fill in before submitting (text area, field, radio button, check box).

      I'm not doing a survey. But then, I guess that the same submission form can be used for multiple purposes.

      What I had in mind was putting the submission form on a PHP or HTML page but will consider putting on a WordPress blog as an alternative. Thanks!

    I don't know about regular pages, but I found a KILLER one for wordpress blogs.

    Custom form plugin for wordpress - I love this and it will do what you need.

    Take a look at limesurvey.org, maybe it helps.

    It allows you to build a survey with all kind of input fields. I didn't look in the admin, but I assume the results can be mailed.

    Take a look and see if it's any good. They have a demo as well.
