Little help from US native speaker/writer

Hi,

I'm not english native speaker/writer and I have to submit some application form in English.
I wrote that form in English, but I need somone to check that for grammar and other errors and to do a little rewrite if needed(to be more like US person writing)

It is about 350 words, I can pay by paypal or cc.

    Yes I know thanks, but there are mainly Indian English and other English speakers and I need just US speakers/writter.

      sbucciarel is about halfway done. You didn't say that it was extremely urgent and I am attempting to enjoy the Memorial Day holiday while getting a little work in on the side.
