Need Help With W-8BEN Form
I have to fill out a W-8BEN form for an affiliate company I'm working with but need help with some of the questions because I'm not very good with tax stuff

I THINK I understand most of it but:
Q8. What reference no. are they referring to?
Q9. I've only checked a - is that right?
Q11. I haven't a clue what that means - do I check it or just leave it blank?
