Looking for a simple survey script. Any suggestions?

So I need a simple survey script that I can embed into a webpage and easily get the results either emails or preferably, delivered via an online report.

(Think= survey monkey)

Any ideas?

P.S - The reason I don't just want to use something like survey monkey is because it's remote... and I need the survey on my page.

    I was just going to say that. I have used survey monkey but one of my friends suggested google docs and not only are they great but free. Can't beat that.

      Vote #3 for google docs if you must have it on your page.

      Google forms can be used as a survey type of thing.

      Personally, I prefer Survey Monkey - but you can't embed with them.
