Looking for a "Probability" Script/Plug-in

Hi guys

Looking for a script/plug-in for Wordpress, one I can edit to my own needs, with questions like:

Do you have website?
With top 10 rankings?
Do you have live chat?
etc etc

IF the user fills 100% YES answers, the Final is a "Wow? Great work", if the user fails some of the answers the final is something like "You need our help.".

Hope it makes sense

Any idea?

    Did find Wordpress Simple Survey, it's a brand new plugin. Testing it right now. Be back after some testings.

    Definitely a keeper, this one is VERY useful for Internet Marketing.

    Surveys can tell us much about our customers/visitors... and can be used to create a NEW sales funnel...

    Visitor follows survey and, depending on the result (I have Good/Medium/Bad) he gets forwarded to a different page...!
