Fail to capture any lead need some help

Hi all Brothers and Sisters,

I needed some help on my blog, i had created my ebook and is giving away for free for my opt in which will appear after 1second after you enter my blog. I am using ulitmate ad footer function.

Just wondering, i had a constant traffic of 50 to 80 visitors per day. But somehow or rather i am unable to capture any lead for 48hours.

Anything wrong with my opt in title or my blog layout etc that failed to capture the lead? Is there anything i can improve?

Seriously need some advise thanks all.

    I'm not sure what I'm supposed to be seeing. I see an opt-in box in the right hand column. You really need to kill the $47 on your report. I had to look twice before I saw the word FREE, the $47 overshadowed it in my mind. (Or at least say "A $47 Value")

    The lead form is not eye catching enough and the call to action is extremely weak.

    Take a look at some popular marketers blogs and see what response pulling graphics and copy they use.
