Need TOS Plugin for Wordpress

I need a terms of service plugin for wordpress, but the one I found at wordpress.org I get an error. I am using 2.6.3 WP. If you have a TOS plugin would you mind sharing the source.

That's the only one I know of. Is that the one giving you an error?
