Does Anyone Know How to Do This?

Hope everyone is having a great holiday season with lots of good food and fun.
Just working on some stuff for the new year and was wondering if anyone knows how to do this below.
I want to create an affiliate resource page where I would provide banners, swipe emails, ppc keywords, etc etc but then have a little "enter your affiliate id" here...like you would enter a Clickbank affiliate code or Paypal email.
Then it would put the affiliate code in the right spot so someone could just copy/paste the email with their code embedded already and either put a banner on their thank you page or send a swipe email to their list.
Does anyone know how to get this "enter your affiliate code" type box to work then put it in the areas where I specify on the page?
